Admission Requirements

Admitted from Home, Assisted Living or Adult Foster Care Home

• History and Physical exam (no more than 5 days old)
• Physician’s orders for admission to Sanctuary at McAuley
• Results of a chest x-ray (no more than 90 days old)
• OBRA screening form (MIMR forms 3877 and 3878 if necessary)
• Health insurance cards (Medicare, Medicaid, Blue Cross etc.)
• Social Security card
• Durable Powers of Attorney for Healthcare, Guardianship, Conservator, Health Care Surrogate (Not required)
• Medicaid Application

A resident cannot be admitted from home without all necessary medical forms. To assure a current History and Physical, an appointment with the patient’s attending physician may be required.

Your family member will be assigned to a physician prior to admission. If a resident’s attending / family physician wishes to be responsible for following the resident at Sanctuary at McAuley, arrangements MUST be made prior to admission.

Federal and Michigan law and rules, require that the resident’s attending physician see the resident within 48 hours of admission, every 30 days for the first 90 days and a minimum of every 30 to 60 days thereafter.
