Medicaid

When applying for Medicaid, you will need at least the following items if applicable:

• Copy of Social Security, Medicare, and healthcare cards
• Verification of all income
• Verification of all assets
• Copy of Vehicle Title
• Copy of the last 12 months bank statements
• Current Deed to your homestead
• Medicaid Application

After you complete the Medicaid Application or if you need assistance, contact the Family Independence Agency to make an appointment with a caseworker.
